Both put a noun in a relational frame before the predicate, but として names the capacity in which the referent COUNTS or acts — action or state alike (医者として働く "work as a doctor", 古都として有名 "famous as an ancient capital", 代表として選ぶ "choose as the representative") — while にとって requires the noun to be the party a stative or evaluative judgment applies to (医者にとって難しい "difficult for a doctor"); neither substitutes for the other.
